COVID Vaccines for 1099 CRNAs

In alignment with Governor DeSantis’ Executive Order 20-315, we are pleased to let you know that we are expanding our vaccination program in a phased approach to include community health care personnel with direct patient contact. In working with our local Department of Health, we have defined health care personnel as licensed medical providers registered with the State of Florida. As more vaccine becomes available, we will work on the next phase of the vaccine rollout where we will vaccinate non-licensed health care personnel such as office staff, and will keep you updated as we begin that phase of the rollout. At this time, this phase is ONLY for licensed medical providers registered with the State of Florida. 

Below we have outlined the process to begin getting your qualified office staff vaccinated.  This is being offered by appointment only, and no walk-in vaccinations are available.

Please read all the information below on how to begin the process to get your qualified staff vaccinated. 

How to begin the process:

• Please designate a single point of contact for your office/practice.  If you are part of a large group with several physicians, please coordinate one point of contact for the practice.
• Have that designated individual send an email to[email protected] to include the following information:

• Name, email address, and phone number for the point of contact for the practice.
• Estimated number of licensed medical providers within the practice.

How do I know who qualifies as a licensed medical provider?

• Every licensed health care provider will be registered with the State of Florida.  License number will be required for scheduling the vaccination.  Below is a link that can be used to verify if you have questions:

What to expect after your designated contact has emailed us:

• We will monitor the email box Monday – Friday and respond within 24 hours or next business day.
• We will provide you with a form to fill out of demographic information for each eligible person that would like to be vaccinated.
• We will use that information to send out to an individual email to each person that will include a link with scheduling instructions including how to schedule, where to go, and all relevant vaccine information. 

Vaccine appointments are limited and will continue to be opened over time as more supply is available.  We look forward to partnering with you to help vaccinate your team.
